RF Microneedling Downtime Day by Day and How to Care for the Skin

Radiofrequency (RF) microneedling downtime is short compared with ablative resurfacing, but it is not zero. The skin has been needled to the mid or deep dermis and heated at depth, so redness, swelling and pinpoint crusting are the expected signs of the healing that produces the result. This guide from The Skin Longevity Clinic sets out the usual timeline after fractional RF microneedling, the aftercare that protects the result and the symptoms that need a review.

How long is the downtime after RF microneedling?

Two to four days of visible redness is typical for scar-depth settings, and superficial settings for texture settle in one to two days. In a 2026 meta-analysis of eight randomised trials, redness after microneedling radiofrequency resolved about 1.7 days sooner than after fractional CO2 laser, and pain scores were lower1. Swelling peaks on day 1, and the pinpoint crusts that form at each needle entry point flake off by day 3 to 5.

When can you wear makeup after RF microneedling?

Mineral makeup can usually be worn from day 2 to 3, once the crusts are dry and there is no weeping. Earlier application risks carrying bacteria into channels that have not yet closed, and brushes and sponges should be freshly cleaned for the same reason.

RF microneedling aftercare that protects the result

First 48 hours. A gentle cleanser and a fragrance-free moisturiser are used, with no active ingredients and no heat from saunas, hot showers on the face or exercise that causes heavy sweating, because heat prolongs the swelling. Sun protection. Broad-spectrum SPF 50 is applied from day 2 and every day for the course, as ultraviolet exposure on healing skin is the main driver of post-inflammatory hyperpigmentation (PIH) in Asian skin2. Actives. Retinoids, glycolic or salicylic acid, vitamin C serums and benzoyl peroxide are paused until the skin is calm, usually day 5 to 7, since each irritates a barrier that has not yet repaired. Crusts. The crusts are left to shed naturally, because picking them leaves marks that outlast the treatment.

Expected and uncommon RF microneedling side effects

Expected effects are redness, swelling, pinpoint bleeding, crusting, dryness and a temporary acne flare in oily skin. Uncommon effects include PIH, recorded as mild in 10 percent of a Korean cohort with skin types IV to V3 and at about one quarter of the risk seen after fractional CO21, prolonged redness beyond 2 weeks, grid-pattern marks that fade over weeks, and cold-sore reactivation in people with a history of it. Rare effects are infection, scarring, and the fat loss or nerve injury reported to the US Food and Drug Administration (FDA) when devices were used outside cleared parameters4. Which symptoms need a review?

A review is needed for spreading redness with pain or warmth after day 3, yellow crusting or pus, blistering, grouped vesicles suggesting a cold sore, darkening that deepens rather than fades after week 2, or a dent in the skin at a treated site. Does RF microneedling downtime differ for acne scars, pores and melasma?

Downtime differs by indication, because each protocol uses a different depth and energy. Scar protocols use deeper, higher-energy passes and sit at the longer end of the range. Pore and texture protocols are shallower and settle sooner. Melasma protocols use pulsed, lower-energy settings with minimal visible downtime, often one day5.

Redness lasts 2 to 4 days for scar settings and 1 to 2 days for superficial settings.

Can I exercise after RF microneedling?

Heavy sweating and heat are avoided for 48 hours, after which exercise can resume.

Can I shower and wash my face?

The face can be washed from the same evening with lukewarm water and a gentle cleanser.

When can I use retinol again?

Retinol can usually be restarted on day 5 to 7, once there is no redness or crusting.

Will RF microneedling cause pigmentation in Asian skin?

The risk is low because the epidermis is spared, but sun protection and settings matched to the skin type are still required3.
